Initial Licensure Online

Attention: Directors and Instructors for Physical Therapist and Physical Therapist Assistant Programs

As of May 15, 2017, applicants for initial licensure will be able to apply online instead of mailing a paper application to the board’s administrative office. Many documents that the applicants submit with the application can be electronically attached and the applicants will be able to pay by credit, debit or e-check.

The Board and its staff encourages all of your students to apply online because doing so causes the application to be received electronically within minutes of submission. If applicants key and submit their own application it minimizes delays caused by the following

 *  Keying errors by staff associated with reading an applicant’s handwriting.

    *  Applicants omitting answers to required questions.

 *   Applicants not signing the application.

 *   Applicants not submitting documents that they can submit at the time of application.

If time is an issue, online applicants can be started then saved for completion later. Applicants have up to 30 days to finalize an application.

Physical Therapists applying online will be able to submit the Mandatory Profile Questionnaire once they submit the application for initial licensure.
