How to Apply for CEUs

How to Apply for CEUs

To Get STARTED:

Select the appropriate application type-- either INDIVIDUAL or PROVIDER

 

INDIVIDUAL APPLICATIONS for APPROVAL OF CEUs:

An Individual PT or PTA may submit an application for approval of CEUs to meet the Tennessee PT licensure requirements. Applications are accepted from individual PTs or PTAs for completed on site or home study/distance education courses. This option is NOT necessary for courses that were pre-approved by TPTA, APTA, or APTA Sections or courses sponsored by a CAPTE accredited physical therapy education program in Tennessee.

 

FEES: INDIVIDUAL APPLICATIONS:

APTA Members $10.00

Non Members $30.00

 

*Application fees are paid online when your course is submitted

*Once your course is approved, you will receive an email notification

*Applications must be completed in one session: incomplete applications may notbe saved

*The assistance of the course provider may be necessary to complete some of the course information

 

INDIVIDUAL APPLICATION CHECKLIST: The following are required for a complete application:

£Course brochure

£Course content and schedule (if not included in the brochure)

£Sample evaluation form

£Copy of letter or certificate of attendance

£Instructor(s) qualifications (if not included in the brochure). Please limit to 2 pages.

PROVIDER APPLICATIONS for APPROVAL OF CEUS:

Continuing Education Course Providers may submit a course for approval

 

FEES: PROVIDER APPLICATIONS:

1-3 Contact Hours $85.00

4-7 Contact Hours $135.00

8 or more Contact Hours $210.00

 

*Application fees are paid online when your course is submitted

*Once your course is approved,you will receive an email notification

Applications must be completed in one session: incomplete applications may not be saved

*Course Re-Approval is not guaranteed.

 

PROVIDER APPLICATION CHECKLIST:The following are required to complete the CEU Provider Application: submission:

-Course brochure

-Course content and schedule (if not included in the brochure)

-Sample evaluation form

-Sample letter or certificate of attendance

-Instructor(s) qualifications (if not included in the brochure). Please limit to 2 pages

-Please Note: All online/Distance Learning Courses must follow the Mergener Formula for calculating CEUS/Contact Hours:

Approved courses will be posted on the TPTA website. Course information will include the title, provider, location, date(s), approved number of hours, and contact information.
